PDF Software (www.pdfsoft.com) offers a PDF eForms OnLine service to capture and store data from a PDF form into a database.

Besides that PDF Software also has developed the Addin XeMailing, a plug-in for Acrobat 5 to merge PDF template form fields with data from a database. It can send the new PDF document by email or save it on disk on a programmed time and date.
