DB Designing problem

I'm having trouble with designing a DB so that it works in a particular situation.

I've created a Word Doc for easier understanding of this problem.

Your diagram and narrative are a bit confusing. Maybe try to clean that up first.

I didn't give the whole model. Maybe that is what is confusing. I'll try to figure a better way to explain
