Can someone point me in the right direction of a good survey/answer database design? I've looked around on here, but there's so much and all mentioning different programmes and I'm lost as to what would be best.

I have to use access and the questionnaire has multiple choice answers where only one answer can be chosen as well multiple choice where up to 5 answers can be chosen. Not really sure where to start!
