I have a database with 7 tablespaces I need to restore from a backup image that has 3 tablespaces , how can I keep my database structure (7 tablespaces) after restore.

You can restore the backup only in the three tablespaces.
If you want you can first restore it in the normal 3 tablespaces, create the additional 4 tablespaces in the restored database. Organize your tables in the tablespaces, then take it's backup and restore it in the database with 7 tablespaces.
