General Index Question

I am playing with some of our older SQL statements and trying to tweak them but little confused by the usage of index. Sometime it uses the index sometime it does not even though columns which are part of the index are part of the sql. So, my question is there are set rule when index will be used and when not. Any link to a document will be highly appreciated.
