Diary Help

I'm trying to recreate a form, and on the current verison there is a Diary area, where the user types some comments in and then when the OK button is clicked it shows up in the Notes box on the form. I trying to find out how to recreate this, since the previous developer did not leave us with this verison of the form..

Any suggestions will be greatful.
