Hello i am ust wonderig if there are any sql forms that will submit to a main database running on sql.

SQL does not have forms, it has DDL and DML statements only

Adding Data to the fields

how do i get users to add data to the fields in the database

an HTML form

perhaps you are looking for how to do the middle step...

users enter data --> application layer --> database INSERT command
