We are building an internet interface for our clients to manage their services and products, however, we plan to incorporate a script on which they can create MySQL DataBases without accessing to cPanel, short of something that works like a form this way:


And once they press Create, the page shows them this:

Does anyone knows a script, paid or not, that is able to generate databases this way, with administration?
