I am a teacher and need to evaluate the use of a website in my teaching.

I want the pupils involved to complete a form indicating their experiences with the website, and then for the form information to be stored on a MySQL database for easy analysis.

I alsready run phpBB on a MySQL database...but don't have much experience of coding.

Could anyone suggest a place to start or some helpful resources?
