Question mysqlaccess giving "broken pipe" error

Running mysqlaccess according to examples on the "--howto" page, but it only gives this "Broken pipe" error (see bottom). All the Google-searches on this error have just said to try easy things like making sure the server is running --it is running.

MySQL version:
MySQL-client-4.0.17-0
MySQL-server-4.0.18-0
MySQL-shared-4.0.17-0 --not sure what this piece does or if we need it. Goal is to get osCommerce working.

OS: Red Hat 9
Linux 2.4.20-8 #1 i686 i686 i386 GNU/Linux
