linking tables

I have 4 tables in my db the customer inserts data from web page I have to break that data into 4 tables because 3 of the tables change often the 1 table that doesnt really change is customer personal info any ideas on how to link this together the web page is http://www.boattruckers.com/form.cfm if this will help with what I'm doing
