How you can track url history in database without making duplicate entry for it

I want to track url history for users of my website. I need to persist it to a table but there should not be duplicates for example if there is a '/' missing it should not create a new entry. What is the best way to do this

Since this is obviously a 3-tier application, is connection pooling used between APP server & DB Server?
