Please help, I'm trying to create a form for a project I'm working on but I've never done anything like this before in my life, I need to set the following up but I've no idea where to start and would be grateful of your help.

I would like to set up a form that I can fill out using my laptop that has a touchscreen, the form would start with the following:

Question 1, Do have a car (only an example)

Yes (if yes is selcted a number of sub questions appear and then when all have been answered then it goes to question 2)

No (if no is selcted question 2 appears)

Question 2, do you have .....

Yes

No

Question 3 , Question 4 and so on.

How would I be best doing this? and what os the best way to start.

This appears to be more of a programming question than a database server question.

Do you have a programming language that you've used before?

Once you've asked the questions, do you need to save the answers somewhere, where they can be retrieved, either by the original answerer, or by others? Do you need reports on the results?
