Cool Entity Relationship models with multiple links berween 2 tables

Someone is going to read this and think "this guys thick" I'm sure bu anyway:

I have an ERM part of which is two entities Fault and User . Each fault is raised by a specific user and closed by the same or another user.

On the ERM should I have two entities with two links or three entities (one "Fault" and two "User"'s with two links?

I was thinking if I only have two entities then surly this would indicate that the fault has to be raised and closed by the same user?

    Would appreciate any help.

    Thanks.

Two entities with two relationships would work:

    Code:
     _______ ______
    | | raised by | |
    | |>---------------------| |
    | | raises | |
    | FAULT | | USER |
    | | closed by | |
    | |>---------------------| |
    |_______| closes |______|
