Question Unanswered: Help in build a Form

    Hi all,
    Wel, i have 2 table's:
    Table1: [ID_P];[Name]
    Table2: [ID];[ID_P];[Date]

    I have this query:
    Q1: "SELECT Table1.Name, Table2.Date
    FROM Table2 LEFT JOIN Table1 ON Table2.ID_P = Table1.ID_P
    ORDER BY Table2.Date;"

    Q2: "TRANSFORM Max("False") AS Expr1
    SELECT Table1.Name
    FROM Table2 LEFT JOIN Table1 ON Table2.ID_P = Table1.ID_P
    GROUP BY Table1.Name
    ORDER BY Table2.Date
    PIVOT Table2.Date;"

I whant to build a form that show the Q2 information in this maner:

    ...............[Date1]......[Date2]......[Date3] And more if exists
    [Name1]..CheckBox1..CheckBox2...CheckBox3 And more if exists
    [Name2]..CheckBox....CheckBox....CheckBox And more if exists

    and so on

If the records dont fit in the form, then the form will have another page.
If the person has a record with a date, the checkbox turn true else false.
My problem is that i dont know how to build this form and associate the field to this kind of query (Q2) becouse the fields are random...
    Plz Help.
    Ty all.

Create a property query for your record source, the create a new form via the Database Window, form tab, selecting the Wizard to buld the form. You'll be prompted with a window for the fields to put on the form and for the new form name.
