Hello all,

I'm trying to create a form that has check boxes to turn "On and Off" fields to display on a subform table and then a command button to export what is visable.

The fields and checkboxes available are:

name
address
city
zip
state
phone

So, when a user clicks the check box on one of the fields listed above, it will be shown on a subform-table.
Also, then be exported to excel.
Does anyone know how this can be done or have a example I can you.
