Hi

I've been asked to create a questionnaire database and am quite new at this. I'm hoping someone will be able to be patience and give me some guidance.

The database is to enable participants to use the questionnarie to assess other participants in the company. Lets say there will be 10 questions and each question will have option buttons associated with it (ie strongly disagree = 5, disagree=4, neutral=3, agree=2, strongly agree=1). Once an option is chosen the value will be displayed to the right of it and once the questionnaire is complete a total will be displayed.

My table so far is as below:

Active (this will be a checkbox as employees come and go. would like to hide them without removing them if no longer employed)

This is where I'm stuck. What are fields for the Response table and which tabel do I include the date in?

Any help would be much appreciated!
