OPEN means the account is not locked AND not expired.

LOCKED means the account is locked. For example, alter user mark account lock. In addition to manually locking the account it can be locked if the profile attached to the user ID has reset the parameter failed_login_attempts from unlimited to a valid value. Accounts stay locked for the amount of time specifed in the profile's parameter PASSWORD_LOCK_TIME, which defaults to unlimited.

EXPIRED means the password has expired and must be changed. For example, alter user mark password expire. In addition to manually expiring the password there are timing conditions set in the profile to control password life times and reusability.

The GRACE indicator means the account has expired but because the DBA has set a grace period in the profile linked to the user's account the user can continue to log in to the database.

The combinations of LOCKED and EXPIRED you listed have the combined meanings of LOCKED and EXPIRED as noted above, so there's no point in discussing them separately.
