    so what have you got so far
    what is your data model looking like
    what reporting and analysis requirements have you got
    has the person(s) requesting this signed off on any specification

    we wont do this for you, we can and will help, assuming its not homework (OR assuming it doesn't look or feel like homework)

    there is a limit to what people can do for you on this site. usually its give a little information, and make a little effort and you will get something (may be a lot) back, give/do nothing get nothing
    I'd rather be riding on the Tiger 800 or the Norton

    Anothers - depends from studies (I don't know what is better buttons or look up again)

    so is the problem in creating the forms to do the data entry or doing the menu

    if you need to create a menu then have a look at the switchboard (tools | database utilities | switchboard
    I'd rather be riding on the Tiger 800 or the Norton

    My first comment is someone needs to download/install Visio.

    Second, my Q's:

    a. Are you using relationships or doing this outside in the VBA?
    b. Are you trying to automate the form creation?
    c. How are you matching this to data backend?
    d. Are you using seperate backend file?
    e. Is this single user or multi-user project?

    Im still not clear on what you are trying to achieve
    are your writing the whole system to to do the data capture, review and analyse the data and report on it or are you adding a front end menu to an existing system.

    if you are designing a whole new system then I think you need to pause and think about what you are attempting to do. if you are starting from scratch and don't have much experience then this forum may not be a good choice, its not really aimed at complete beginners, its more aimed at people currently developing stuff and hitting probelms, rather than going through each and every stage to get a solution together. that doens't mean we can't and won't help, its just there is not enough time for any one person to mentor you through the whole process, and receiving different bits of advice form different people can be very very confusing, as often there is no 'one' true way of doing things in the systems world, theres often many right solutions, and masses of wrong solutions, but its difficult to mix and match different bits from different approaches.

    there are some excellent sites (and books) on user interface design, you may even find some here

    if you are involved in designing the data store then these two sites give a very good groundign on data design
