Do you mean the Oracle services are up & running, or the database is up & running? (You can have Oracle running without any databases needing to be open.)
90% of users' problems can be resolved by punching them - the other 10% by switching off their PCs.

If the database is down, then how can anything check "at DB level"?
What problem are you really trying to solve?
How would independent observer know when problem has been solved?

The database can be up without the listener running.
    SQL*Net <> DB

You can check at the Operating System level.
