    When do we create clustered index on a table?
    (I am not asking the difference between clustered and non-clustered)

    Almost always. Unless you have a very compelling reason for your table to be a heap then you should always have a clustered index.

    If all records in a table are expected to fit 1 page (8060 bytes), like in the case of many lookup tables - there is not need for a clustered index.
