    Unanswered: field where you can select dates and store as multivalued field

    I have a table of unique ids which stores patient names and last names, in another table that's linked to the first table that has start date of service and end date of service for each unique id. what would be the best option for me so that inside the second table each field can have multiple dates for specific service done within the Date of service(DoS) start date and DoS End date.

    Example
    Level 1
    Id Name
    1 John doe
    Level 2
    Dos Start 11/25/2013
    Dos End 11/30/2013

    Level 3
    Service A done
    11/25/2013
    11/26/2013
    11/28/2013
    11/30/2013

    bold text are separate fields

    Are those the only fields in the table(s)? If so, then you would need another table for Service Types.

    there are about 13 different services that can happen at the same time. I figured that if i get one of the service tables i can get the others.

    right now i have the level 2 linking with level 1 quite well, but i'm getting hung up trying to link level 3 with 2 and 1

    Im guessing 'level 1' is a table defining persons
    'level 2' is a table that defines when contracts for persosn start and end
    'level 3' is a table that defines actual service calls for the contracts in Level2
    I do hope you aren't contemplating having 13 different service tables..... please say it aint so
    Id expect another table to identify types of services
    I'd expect another tabel which identified whats services are offered to each customer for what period, and then from that what is your current level 3 stores the type of service and date of service
    I'd rather be riding on the Tiger 800 or the Norton

    you were right about level 1 and 2

    Level 3 I might have been thinking of 13 different tables.

    Level 3 is within the dates defined by their stay in level 2
    the resident could have different services provided each date within those dates. We have about 13 services that would be done each day.

    What i need is to be able to output to text delimited

    so it spits out residents
    unique id 1, service A, done on this date
    unique id 1, service B, done on this date

    i'm a little stumped on this third level and efficiency .. it use to be that i just need the start date of service and how many units of the service they got during that span. But we needed more accuracy.

    how can i change that so the person can have multiple service spans as well

    for example

    person 1 gets

    Service Start 1 11/01/2013
    on service date 11/01/2013 person gets service A
    on service date 11/01/2013 person gets service B
    on service date 11/03/2013 person gets service A
    Service Ends 1 11/15/2013

    Service Start 2 11/20/2013
    ect
    Service Ends 2 11/30/2013

    by adequate physical design of the tables and rows so that the design meets the business requirement.
    based on the model in post#8 you'd define a new contract for the person...

    I'm having trouble putting together the linking of services type and contracts. Can you explain the table of service types and how it relates to contracts
