Unanswered: Slow Changing Dimension Types 1/2

I understand the difference in Type 1 and Type 2. My question is the implementation. I've been unable to find a clear, simple example of how to declare a Type 2 dimension table.

Is it as simple as having through dates (ie. start date / end date)? If I were to look at a table description, how could I tell if it is Type 1 or Type 2?
