Unhappy Unanswered: Number Of Index' Tuples (21539) Is Not The Same As Heap (18607)'

I received this error during a Vacuum and after this I obtained a duplicated primary key.

Some one know what does mean?

I see, but usualy after a post aftwer a few minutes I receive the email ( trought the mailing list ). May be I'm a little bit nervous due the fact that we are managing a service that should be UP 24/24 7/7 and after using Postgres 7.3.2 we are having a lot of problem:
duplicate primary key and a SIGSEGV ( signal 11 ) caused always ( but not all the time ) by the same select ( the select is performed like 5000 time in a day ) and usualy happen once a week; after receiving the signal 11 all connections are dropped :-(

    Thank you
    Gaetano
