Unanswered: Want to open a second form based on 2 form fields from form 1?

I have a form, startup, that has a name field and a date field. The name field is a drop-down box that lists all the names currently listed in the name column in the table. The date field has got an input mask on it to make sure they enter the date correctly. How do I open a second form based on the info entered/selected into the first forms 2 fields? Thanks for your help.

Using the query (recordsource) behind the second form
place the following in the criteria row for each appropriate field

    Field1 (Columnhead)
    Forms!form1name!myfieldname 'criteria line

    Field2 (Columnhead)
    Forms!form1name!myotherfieldname 'criteria line

Then from form1 use a commandbutton to open the form and the appropriate record(s) will be queried.
