    Unanswered: Progress Form

I really need help on this one, I've been searching the web for a while with no use. I'm not really into server side programming or creating complicated forms. I use Dreamweaver MX for simple html pages.

    My question is:

How can I create a Form with 5 fields for example and whenever submitted it will update to the database and show the results on the same page or a new page. And whenever I need to update any of the submitted reports or information I can easily go to that page and just edit the old items. And also submit new information.

Maybe Im not too clear about my requirements but there's a CGI script online which is very similar to what I need but it does not archive anything to a database. Check it out >> http://stein.cshl.org/WWW/software/C...ples/tryit.cgi

My hosting server supports the following:

    -cgi-bin with Perl 5.6x
    -PHP 4.3.1
    -ASP
    -ASP.NET 1.1
    -ColdFusion
    -MS Access
    -MySQL
    -MS SQL Server 2000

Any help is very much appreciated..

    numLocked.

Thank you so much your script will do the job perfectly and I would be grateful if you can send me the source and tell me what sort of database it requires, MS SQL or MySQL?

I was also wondering if I can create a logon page so that certain individuals can access and modify the records, and others can only view it. Let me know if you are familiar with that.

    Later...
